All are responsible. Risks can come from anywhere - Like testing issue may get some showstoppers and it may cause delay. The programmers and tester can identify such risks.
XP coach may observe risks due to some team disfunction. On-Site Customers may identify risks related to external environments which may introduce risk to deliver value. In summary all roles work together for effective risk management.
It is not limited to a particular role.
