A project was about to get delivered but got stuck with an issue at the final stage .This is resolved by the team member that added a good functionality to the project but they did not perform the integrated change control process.

I will go with option B, raising this as an issue that process was not followed and then take next steps

So , you mean we raise an issue and close it as we need to notify genuinely .. ? or is there any other procedure to handle these situations.

From the sentence about to be delivered, we can think this happening during Control Quality Process( Verify Deliverable -issues are found and resolved) and before Validate Scope(deliverable accepted/signoff).
During quality control, if any issue found a defect/issue is raised and during defect addressing, we fix the issue if its not violating baselines(scope, schedule, cost, quality). If fix violating the baselines, we raise the CRQ and due process to complete the CRQ.
If its time to go for Validate Scope before fixing the issue/CRQ, during the discussions with customer, it needs to be appraised.
