Monitor Risks

If we identify a risk in monitor risks process then we have to do the prioritization process (Qualitative and if required then quantitative analysis), plan the risk responses, and implement the risk responses in the project management plan.

My query is that the above mentioned entire risk management process would fall under monitor risks process or would we go back to identify Risk process and follow other process mentioned in the planning and executing process group? Essentially meaning that we come back to the planning, executing processes of risk management and we do not remain in the monitor risks process any more?

Please help understand.

Risk identification, quantifying it etc is an ongoing process

Essentially meaning that we come back to the planning, executing processes of risk management and we do not remain in the monitor risks process any more?
YES for brief period and then return back to monitor risk

Please help understand.
