So far my understanding requirement documentations are all of the documents needed for the project and
Requirement Traceability Matrix is a grid or table where all of the requirements are listed from the beginning of the project till the deliverable of the product in Matrix or Tabulated format could be updated during different phases of the project. Both of those are outputs of collect requirement process.

Manish can explain more!

Requirement document - usually a detailed text based document which captures all requirements (through various requirement gathering t and T from all possible stakeholders that are identified by the project) . This documents focus is on mapping the requirements to projects business case, detailing the requirements as understood from stakeholders. So you can assume focus is on origination of requirements. nce the scope is baselined (there could be many requirements which are not included in the scope baseline) then the requirement documentation helps to understand why a certain requirement was in and not in the scope baseline if there are any queries from any stakeholders in future,.

RTM - This a matrix, to trace the requirement from its origin to its conclusion. as name suggests the trace-ability is to track when each of these requirements are completed, by who, when and who approved the deliverable(s) corresponding to the requirement. RTM focusses on downstream not so much on who gave the requirements but on where the project is in terms of completion of the deliverable linked to a requirement
