I think cancel the project is the least likely to have but at the phase end review, it happens.

According me Option is D is correct, Cost management plan is part of Planning process group and budget can't be measured at each and every phase review, Hence Option D Correct.

Manish, Correct if i am wrong.

lets understand close phase -

It is a checkpoint used to validate the completion of the phase, hand over deliverable to operations at the same time is also called as kill point, as the name suggest it is a check on projects business case relevance and if there is a need to kill the project or move on with subsequent phase.

now let understand in what situations do we need additional budget - when there is change in any of the parameters (scope/schedule) which will need additional budget.
IN close phase the discussion is more focused on business case validity of the project and formal completion of a given phase.
When the next phase begins there could be changes during the planning phase impacting the budget.

So while there is a possibility that budget change may happen during the closure of a phase, it is less likely as compared all other options provided

Justification is understandable but question has asked for least likely option. Phase end review rarely cancelled the project. Try to mitigate or change anything in terms of budget , scope or schedule etc.

As I mentioned above Phase end review is also called kill point and that means a point to cancel the project if business is not valid anymore. So Project cancellation can happen.

project cancellation is not least likely option for sure.
