I will choose option 2. Incomplete project management plans
Project Charter is high level documents and don't have detailed deliverables or schedule baseline.
Although incomplete scope management plan can also have ripple effect in changing deliverables and schedule baselines.

The best possible Option is the Option # 1 1. The lack of clear instructions from project sponsor. The other options follow thereafter.

Its not necessary to match your thought process with each mock test, finally the PMI is the one who is gona take final call , each writer (including me
) may have some different view for things
Change in deliverable mean unstable scope, which in turn resulting in change of schedule, since it is getting baseline which shows PM and team is doing replanning and sponsor is approving the replanning again and again.
Now look for options
Could be the reason that is why he is ok for replanning.
I am not getting any clue where it shows that project management plan was incomplete and that is resulting in change, rather question is saying we have baselines

This could be the reason since the change is happening in scope , but since we have approval of baseline i prefer selective option 1.
