Approved Iteration.

Iterations are approved based on many factors like:
Priority of features
Available time to market and
Team Capacity
If Scrum approach is used, Product Owner makes sure iterations are approved as a result of collaboration with stakeholders and development team.
Anyways whatever approach is used, negotiations and collaboration are needed to the desired objective.

In case of agile we focus on learning for mistakes, assuming you end up in a situation where team gives a demo to stakeholder and Product Owner and they find the content of iteration not good enough. So what does it mean , it mean you did not met the iteration goal, what you do about it? you discuss it in the retrospective meeting, team along with Product Owner need to find what best case be done to avoid this happening again.
