Information required for claiming Category A SEU

Attended the webinar "Testing Strategy for Agile Teams" on 12th Mar.
Need to claim the 1 SEU we would have got for this webinar.
This needs to be entered as Category A event , however have a few questions wrt few fields

Could you please help with those:
1) Entered sub category as "3 - Scrum Alliance User Group Activity". Is this correct.
2) Value to be entered in following fields
- Event Organizer or Scrum Alliance Endorsed User Group Name
- Event Organizer or Event URL
- Scrum Alliance Endorsed User Group Main Group Owner (required if user group event) Name
- Scrum Alliance Endorsed User Group Main Group Owner (required if user group event) Email
