In general (untill question reflect something special) Project Manager is responsible for ensuring all 47 Processes of PMBOK performed well. Project Integration Management knowledge area is many time considered the primary role of PM since PM need to ensure everything remains integrated to achieve the project objectives. So PM is definitely responsible for Integrated Change Management.

Project Manager is Responsible for All processes (47) from inception to completion,
Ref : PMBOK 5 Page -96 ; Ist Paragraph.

Change Management such as change request either to be approved or rejected is Project Manager or Project sponsor is ultimately responsible.
PMBOK -5 ,page -96 3rd paragraph.
