Before you define project scope - you need to collect requirements from Stakeholders(Sponsor/Customers etc); Stakeholder Register (output of Stakeholder analysis tool in Identify Stakeholders) acts as input for collecting requirements
B.When developing the project charter (project charter acts as input for identifying stakeholders)
C.When creating the project management plan
D.When putting changes through change control (its part of monitor/controlling process group)
I am confirmed B and D are definitely not possible.
