PMBOK says the process "Identify stakeholders" is repeated as necessary, but SHOULD BE performed at the start of each phase of the project.
Could you please clarify this point.

Project needs to constantly update stakeholder list, identify new stakeholders, delete stakeholders if they are no longer relevant but also update their category/ classification.
This is an ongoing project however it becomes more important at the start of each phase to ensure that we have the right stakeholders identified because at start of each phase project also revisits business case
