Tracking Deffered or Cancelled Requirements

The client may have a lot of requirements which we did not implemented or accepted in the solution

I have 2 questions :

1-Do we keep tracking the rejected requirements using Requirement Traceability Matrix cause maybe the stakeholder come again with a change stating them again so we need to know that previously we did not accepted them?
I believe in RTM we can add Status, such as active, approved, deferred, cancelled

2- What happen In case it has been cancelled or deferred do we need to perform a request for change to CCB ?

In RTM if the status is maintained deferred, then no action will be taken untill it's status become active.
