We can claim the SEUs in Category "E" by reading a book. This category have the section for "Activity list". Do I need to add the chapter details of the books here ? Please suggest.

Providing chapter details wont be any help. There should be a learning objective from your point of you. Also, you need to provide activity description, it has to be overall learning from the book. You can add more information about the stated objective of the book written by the author himself. You can find this information on book preface and introduction area. Additionally, the learning objective would be crystal clear to you when you read the complete book.

For the "How has this course or coaching helped you develop as a Scrum practitioner?" column:

Think of following questions while writing this section -
1. How the new knowledge gained from the book relates to your current scrum experience?
2. How the new knowledge gained from the book helped you grow as a Scrum Master?
3. What actions/Practical implementations you will do after gaining this new knowledge?
4. What actionable insights you shared with your Scrum team from the new knowledge?
