A business case describes the problem or opportunity in hand, what were the solutions considered and which one was chosen, what problem does this project solves (benefits), how Business are going to make money, how much time will it take to recover cost and make profits etc. In other words, business case summarizes if Business should put money into a project.

Business plan on the other hand describes what are the Business analysis activities will be done, how requirements will be elicited, how stakeholders will be identified, which documents will be produced as a part of deliverables, who will be taking decision in case there is a change in requirement or change in scope etc.

A business case may or may not start a Dev project whereas a business plan shows that the project has started.

Just to add:

Business Case: A documented economic feasibility study used to establish the validity of the benefits of a selected component lacking sufficient definition and used as a basis for the authorization of further project management activities.

Business Case includes following:
Problem/Opportunity: A situation statement, relevant data to assess the situation and identify which stakeholders or stakeholder groups are affected.
Analysis of the Situation: Organizational goals and objectives are listed and also Include root cause(s) of the problem or main contributors of an opportunity. Support the analysis through relevant data to confirm the rationale. Include needed capabilities versus existing capabilities. The gaps between them will form the program or project objectives.
Recommendation: Present results of the feasibility analysis for each potential option. Specify any constraints, assumptions, risks, and dependencies for each option. Rank-order the alternatives and list the recommended one; include why it is recommended and why the others are not. Summarize the cost-benefit analysis for the recommended option. Include the implementation approach, including milestones, dependencies, roles, and responsibilities.
Evaluation: Include a plan for measuring benefits realization using metrics to evaluate how the solution contributes to goals and objectives.


Business Analysis Plan: A subplan of the project management plan that defines the business analysis approach, including the tasks that will be performed, the deliverables that will be produced, the roles required to carry out the process, and process decisions regarding how requirement-related decisions will be made; how requirement priorities will be set; how changes to requirements will be proposed, approved, and managed; how requirements will be validated, verified, monitored, and traced; and how business analysis communication will be performed.

In my view Business Plan and Business Analysis Plan are two separate thing.

A business plan is a formal statement of business goals, reasons they are attainable, and plans for reaching them. It may also contain background information about the organization or team attempting to reach those goals.

Business plan is about starting up business or an organization for some goal.... where as business case is business justification of doing projects or initiatives in business.
