Manish sir pls help--CR versus project document updates

many processes have output as CR, mostly control and execution processes and only one from planning (plan Procurement mngmt).

CR ultimately leads to Project management plan (relevant subsidiary plan) update.
ideally this way...

Process-->Raise CR--> Perform Integrated Change control --> Approved CR--> Update Plan.

my question:
lets take case of plan risk response process. this triggers changes in project management plan then why not is this done through formal change request management??

random thoughts in my mind:
because Project mngmt plan is not yet approved/frozen yet?

if my thoughts are making sense then why CR as a o/p of plan procurement when it might be the case that project mngmt plan is not yet approved in this case as well?

Yes thats true in case PMP is not baselined and approved we do not need a CR to update the plan,
do remember that since the process is iterative, plan risk response could also happen after the PMP is baselined and then in that case there is a need of CR to change baseline plan of PMP

Again plan procurement can also happen after PMP is approved, as a decision to procure can happen anytime in the project

i got another or may be same query on same line while re-visiting risk management area:

i interpret that PMBOK ITTO table assumes that when we are doing risk management at first then scope baseline is ready but no other baselines are ready yet.

Plan risk response says that due to this risk planning there will be "project documents update" like scope baseline (PMBOK ,Page 347). my question is updates to baseline should be considered only through CR? if so then plan risk response process should generate CR? if it should generate CR then why is there no mention of CR as output of this process in pmbok?

i am not able make peace with whole cycle here.. if someone can illustrate an generic example of risk , plan risk response, project document update process then it would be great help..

There is no prescribed sequence however it makes sense to do risk response, quality planning , etc before baselining

In first pass when the scope is not baselined we do not need a CR however if PM is doing risk plan risk responses after baselining and there is impact on any of the baseline then we will need a CR and as I have mentioned many times before PMBOK is not a rulebook its a guide, so if one of the output is missing , it does not mean a PM cannot generate the output through a given process.
