Kshitij,

I feel definition wise you are clear that check sheet is basically used to organize facts in a manner that will facilitate the effective collection of useful data about a potential quality problem. So this is something to do with organization of data collected in a way which can make sense and help in resolving problem.. example can be quality document coming out of DP meetings in Organisation.. have a look below
Cause Frequency
Violation of coding standards 9
Lack of domain Knolwdge 5
Lack of self review 6

checklist is a structured tool, usually component-specific, used to verify that a set of required steps has been performed. Checklist is always pre structured or organized with a specific interntion. example code review checklist for reviewer so that some common checks are not missed

1. indentation is followed as per coding standards
2. comments are present for each module/paragraph of code

Please get back if there is still a doubt
