An authority within the project management team or external organization.
The PMO
The stakeholder who raised the change request.
The project manager or Sponsor.
From Izenbridge the answer is Project manager and sponsor . I agree to that , but from SCODO it is

usually it cannot be one person approving the change it has to be a body If I am forced to choose will go with option #1.
