The answer should be "3", they are in validate scope process where formal acceptance of the deliverable is obtained and that happens whenever a deliverable is completed after quality verification.

I think the answer is option A, since it is a decision that a PM wanted to take so analyzing the implication of a particular alternate. Option D, says the decision is already taken and analyzing the impact later.
