Many books and PMBOK talk about basic differences on Organizational structure. But don't see depth of discussion anywhere. From the PMBOk diagrams on Organization types, we see PM reports to Functional Manager in Strong Matrix. And in projectized PM is directly reporting to a separate manager of PMs...

PMO might also be managed closely by PM depending upon the type of PMO and importance of project to the Org.

PMO (in some cases) could be part of CCB.

Does this statement holds true?

You may be wrong. But would like to know the view points here and adherence to PMBOK.
Please elaborate bit more on why you think "Issue Log" is the main input for Manage Project team process?
