Every change in the contract scope has to be formally recorded irrespective of it is due to law, or not and if its urgent or not.
Hence its essential to open up a change control mechanism

lower level grids are used within the group to designate roles, responsibilities and level of authorities for specific activities. Within the groups there could be sub groups as well. The lowest level grid has only individuals. however to keep it simple and not confuse students I would agree with you,

agree project charter is needed, you dont need to do any reverse engineering but project charter is the document which creates legitimacy for project and PM, not project plan.
